Congratulations global service-learning participants! As you prepare for your exciting journey in service-learning, there are a number of important paperwork steps and deadlines you must complete. Each step takes you one step closer to your journey...

Plan Your Trip


Step 1:  Program Enrollment Confirmation

Within two weeks of receiving your program acceptance email, please complete the following to confirm your program enrollment:

  1. Read the AGSLC Program Information Packet (pdf)
  2. Sign and Return your
    Amizade AGSLC Liability Form (pdf)
  3. Submit a $250.00 non-refundable program tuition deposit and the accompanying AGSLC Payment Information (pdf) form.  This deposit is deducted from your overall program tuition fee. Checks made payable to Amizade.

Step 2:  Contact Your Study Abroad Office

Non-WVU students: Submit a signed copy of the WVU Transcript Request Form >> (pdf)

WVU students: Bring a copy of your acceptance information to G-13 White Hall.

Step 3: Review and Complete Your Program Participation Requirements

  1. All students will need to read, sign and submit the following documents to Amizade before their program start date:
  2. Non-WVU students will need to submit a signed copy of the WVU Transcript Request Form >>   (pdf)
    NOTE: WVU Full & Part-time students do not need to complete this request form 

  3. All students will need to submit the following information to Amizade before their program start date:
    • Copy of Passport & Country Visa (where applicable)
    • Confirmation of Required Immunizations
    • Copy of airline flight itinerary (if individually arranged)

Step 4: Complete Your Travel Requirements

  1. Passport
    All students traveling out the United States are required to secure a passport.  Information on how to obtain a passport is located in the AGSLC 2007 Program Information Packet. 

  2. Country Visas
    Students traveling to the following countries are required to obtain a country visa:
    • Brazil
    • Ghana
    • Tanzania

    NOTE: Other countries where AGSLC programs take place will allow you to visit without a visa for certain periods. If you are planning on staying longer than the scheduled AGSLC program dates or have other special circumstances, please visit the US State Department website for visa information at:http://travel.state.gov/foreignentryreqs.html


  3. Health & Immunizations
    Participants must comply with the Centers for Disease Control (CDC) travel and immunization recommendations for the locations you will be traveling.  Once enrolled into a program you will receive information from AGSLC on the specific immunization requirements for your program.

Step 5:  Make Your Airfare Arrangements

Group flights  
Group flights will be announced to accepted participants during March-April 2007 depending on when courses fill.  You will have 1-2 weeks to confirm your ticket on the group flight and make payment to the travel agency.

Individual flights
If you decide to book a flight on your own, you must be at the airport within 2 hours of the group flight arrival time.  You must provide Amizade with a copy of your flight itinerary before or by May 1, 2007.
